Is designing extra logo concepts out of scope for a web design project?
Usually yes, once the agreed number of concepts has been delivered and a direction chosen. Additional concepts beyond that are a new exploration round, priced the same way any revision-limit overage is.
The proposal named a specific number of initial logo concepts — often two or three — plus a set number of revision rounds on the chosen direction. The client picks one, revises it within the included rounds, and it gets finalized. Sometimes, before or even after that point, the client asks to see "a few more options" — a fair ask in the abstract, but one that goes beyond what was priced if the concept count was already used.

Usually yes, once the agreed concept count has been delivered. Refining the chosen concept within the included revision rounds is normal work; generating entirely new concepts beyond the agreed number — whether before a direction is chosen or after one already was — is additional exploration outside what the fee covered.
